Welcome to Walk In Medical Center! We're excited to have you as a new patient. Here's what you need to know for your first visit.
Before Your First Visit
Please Bring:
- Insurance card - Both front and back
- Photo ID - Driver's license, passport, or state ID
- Current medication list - Include dosages
- Medical records - If available from previous provider
- Allergy information - Drug and food allergies
What to Expect
Registration: Plan to arrive 10-15 minutes early to complete any necessary paperwork. Our friendly staff will help you with the registration process.
Medical History: You'll be asked about your medical history, current medications, allergies, and any health concerns you'd like to discuss with Dr. Nadeem.
Comprehensive Exam: Dr. Nadeem will perform a thorough examination and discuss your health goals, preventive care needs, and any treatment plans if necessary.
Questions Welcome: This is your opportunity to ask questions and establish a relationship with your new family physician. We encourage open communication!
